The charge of this project is to provide an ongoing stream of methodology to improve hospital quality of care and patient safety systems. WVHA Patient Safety Council will initially focus on a few projects. As each of these projects are completed, new projects (needs) will be identified by the hospitals. WVHA is committed to helping our members deliver safe, high-quality health care to patients in all clinical settings. Despite the best intentions, however, a high rate of largely preventable adverse events and medical errors occur that cause harm to patients. Adverse events and medical errors can occur in any health care setting in any community. Through WVHA's efforts to improve patient safety, West Virginia hospitals now have access to a coordinated, extensive set of tools designed to reduce medical errors.
